Application forms
The DNR has developed these application forms to help streamline the approval process and reduce the amount of time it takes. The forms will walk you through the specific administrative code requirements that apply to your facility or activity.
Before completing a form, contact the DNR waste management specialist for the county in which the facility will be located.
Submit completed applications to the Waste and Materials Management Program regional supervisor for the county in which the facility is located. Use the map to find the correct supervisor, and the DNR Staff Directory for mailing address information.
