RR Program Submittal Portal
Submit forms and documents related to ch. NR 700, Wis. Adm. Code.
Submit Electronically
Customers are strongly encouraged to submit documents via the online Submittal Portal. The DNR is temporarily suspending the requirement to submit one paper copy of each plan or report under s. NR 700.11(3g), Wis Adm. Code, including hard copies of case closure packets.
The Notification for Hazardous Substance Discharge Form – Non-Emergency Only (Form 4400-225) is now a web form accessible through the Submittal Portal.
Compliance with NR 712
Professional Engineers are permitted to certify reports using a digital or electronic signature and P.E. stamp for any submittal subject to ch. NR 712, Wis. Adm. Code, requirements. Hydrogeologists, professional geologists and scientists may certify using a digital or electronic signature for NR 712 submittals. (See ss. A-E 2.02(7) and GHSS 1.03(7), Wis. Adm. Code, and s. 137.15, Wis. Stats.)
The RR Program Submittal Portal allows external customers to easily submit electronic copies of forms and documents required by chs. NR 700 - 799, Wis. Adm. Code. Within the portal, the Document Uploader is available to submit electronic copies of documents related to the assessment, investigation, cleanup, closure and/or redevelopment of contaminated properties.
Document submittal requirements are listed in s. NR 700.11(3g), Wis. Adm. Code, as well as throughout chs. NR 700 - 799, Wis. Adm. Code, for specific situations. Responsible parties or their representative must provide the DNR with one paper copy and one electronic copy of all submitted documents, including those related to case closure requests. The electronic file should be an exact duplicate of the paper document.
The Notification for Hazardous Substance Discharge Form – Non-Emergency Only (Form 4400-225) is now a fillable web form accessible through the Submittal Portal. Use this online form to report discharges that are identified through laboratory analysis of soil, sediment, vapor, indoor air and water. Additional web-based forms, including the ability to pay fees online, will be added to this portal in the future. Note: When web-based forms are made accessible in the portal, the PDF versions will be archived and no longer available.
Access the Submittal Portal, Document Uploader and Web forms
- Get a Web Access Management System (WAMS) username and password if you don't already have one. See the directions for getting a WAMS account [PDF]
- Go to the RR Program Submittal Portal application and log in with your WAMS account.
- Click on DNR RR forms in the upper right.
Uploading documents
- Click “Start” in the Document Uploader box.
- Complete the Document Uploader form, attach documents and submit.
- FIRST-TIME USERS: Complete the required portal access request form to receive an email granting you access to the portal. Allow one business day for the access email from time of submission.
- If you do not receive portal access approval within one business day, please contact Danielle Wincentsen for assistance.
- Partially completed forms can be saved and finished later. Find saved forms using the “In Progress” link.
Filling out and submitting the Notification for Hazardous Substance Discharge Form – Non-Emergency Only (Form 4400-225)
- Click “Start” in the box labeled Notification for Hazardous Substance Discharge Form.
- Information from your WAMS account will auto-populate in the Reporter tab. The first time you fill out this form you must input your company name. This information will then be saved in the system and will appear the next time you use the form.
- Partially completed forms can be saved and finished later. Find saved forms using the “In Progress” link.
Adding lab reports and other documents at a later date
- The Submitted forms – pending attachments section of the Portal home page can be used to store forms when lab reports and other documents were not initially available.
- To use, navigate to the last tab, upload your lab report and/or other documents and submit the form. An email will be sent confirming that your form is submitted and will be saved under the “Submitted” link on the Submittal Portal home page.
